The Gilbert Centre is committed to protecting the privacy and confidentiality of your personal health information. We adhere to the Personal Health Information Protection Act, 2004 (PHIPA) and other applicable laws in Ontario. 

ELECTRONIC MEDICAL RECORD: ACCURO

The Gilbert Centre participates in a community-wide Electronic Medical Record (EMR) system, ACCURO, shared with the Barrie and Community Family Health Team and other Family Health Teams in the Simcoe/Muskoka area. This allows authorized healthcare providers involved in your care to access your information to provide you with the best possible service.

Safeguards are in place to protect your information:

  • Maintaining a secure electronic security environment
  • Protection against theft, loss, and unauthorized access, including audits
  • Continuous education and training staff and clinicians on privacy;
  • Minimum information collected for your care

COLLECTION

When you receive care from the Gilbert Centre clinic, we collect personal and health information about you directly from you or from someone with the legal authority to represent you. The information that we collect may include your name, date or birth, address, health history and current conditions, and records of your visit and the care that you received. We will only collect the amount of information that is required to provide you the care that you need.

USES & DISCLOSURES

Your personal health information is used and disclosed for the following purposes:

  • To care for you and improve your health;
  • BIll and receive payment for the care we provide (OHIP and WSIB)
  • Improve the quality of the care that you receive
  • Conduct risk management activities and improve our programs and services
  • To support teaching and staff training purposes
  • To comply with legal and regulatory requirements
  • Conduct clinical research, in accordance with established ethics and privacy guidelines;

YOUR RIGHTS & CHOICES

You- or your authorized personal representative- have the following rights regarding your personal health information:

  • Right to request access: You can request to see your personal health information.
  • Right to request corrections: You can request to have any inaccuracies in your information corrected.
  • Right to withdraw consent: You can withdraw your consent for certain uses and disclosures of your information.
