Working Together to Improve Access to Specialized Care
The Gilbert Centre Clinic values collaboration and welcomes referrals from healthcare providers, hospitals, public health, mental health and addictions services, shelters, outreach programs, community organizations, and other service providers throughout Simcoe Muskoka.
Our team provides specialized HIV, Hepatitis C (HCV), and sexually transmitted and blood-borne infection (STBBI) clinical services, working alongside our partners to improve access to care and reduce barriers for the communities we serve.
Who Can Be Referred?
We welcome referrals for individuals who may benefit from specialized clinical services, including:
• HIV prevention and clinical care
• HIV testing
• Hepatitis C (HCV) testing, assessment, treatment, and follow-up
• STBBI testing, treatment, and clinical follow-up
• PrEP (Pre-Exposure Prophylaxis)
Our Assessment Process
Every referral begins with a clinical assessment to determine the individual's needs and identify the most appropriate services.
Based on the assessment and program eligibility, clients may also be connected to additional supports available through the Gilbert Centre Clinic or our community partners.
These may include care navigation, education, counselling, peer support, or referrals to other health and community services, where appropriate.
