Hepatitis C (HCV) Services

Early diagnosis and treatment can cure Hepatitis C and improve long-term health outcomes.

• Linkage to healthcare and community supports

STBBI Services

We provide confidential services related to sexually transmitted and blood-borne infections.

Services include:

• Testing
• Treatment
• Education
• Follow-up care
• Risk reduction education
• Referrals to additional healthcare services when appropriate

PrEP (Pre-Exposure Prophylaxis)

PrEP is medication that helps prevent HIV before exposure.

Our team can help you:

• Learn if PrEP is right for you
• Complete the required assessments
• Start treatment
• Receive ongoing monitoring and follow-up

Linkage to Care

Accessing healthcare can be challenging. Our team is here to help.

We can assist with:

• Navigating the healthcare system
• Connecting with healthcare providers
• Referrals to specialists
• Connecting with community services
• Care coordination and ongoing support

Social Work Services

Our Social Worker provides compassionate support to help clients address challenges that may affect their health and well-being.

Services may include:

• Individual counselling
• Psychosocial support
• Crisis intervention
• Advocacy
• System navigation
• Referrals to community resources

HIV/HCV Peer Support

Our Peer Support Worker provides support grounded in lived and living experience.

Peer support may include:

• One-on-one peer support
• Education
• Practical assistance
• Emotional support
• Advocacy
• Navigation of health and community services
• Connection to community resources

Health Education

We believe knowledge empowers people to make informed decisions about their health.

Our team provides education on:

• HIV
• Hepatitis C
• STBBIs
• PrEP
• Harm reduction
• Safer sex
• Safer substance use
• Preventing transmission

Outreach & Community Engagement

Our multidisciplinary team is committed to providing specialized HIV, Hepatitis C (HCV), and sexually transmitted and blood-borne infection (STBBI) services in a welcoming, low-barrier environment.

Refer a Client

We welcome referrals for individuals who may benefit from specialized clinical services. Every referral begins with a clinical assessment to determine the individual's needs and identify the most appropriate services.
