Meet the Health Program Team

At the Gilbert Centre Clinic, our multidisciplinary team is committed to providing specialized HIV, Hepatitis C (HCV), and sexually transmitted and blood-borne infection (STBBI) services in a welcoming, low-barrier environment.

We believe that everyone belongs, and everyone matters. Every person who walks through our doors deserves to be treated with dignity, compassion, and respect.

While each team member brings unique expertise, we share a common goal: to provide compassionate, client-centred care that reduces barriers, promotes health, and connects people to the services and supports they need. Together, we support clinical care, outreach, health promotion, education, community engagement, and strong partnerships to help improve the health and well-being of our communities.

Health Program Manager

Provides strategic leadership for the clinic by supporting program development, quality improvement, and community partnerships. The Health Program Manager works collaboratively with the clinic team and partner organizations to strengthen access to specialized HIV, HCV, and STBBI services while advancing outreach, education, and integrated care.

Nurse Practitioner (NP)

Provides specialized assessment, diagnosis, testing, treatment, and follow-up care related to HIV, HCV, and other STBBIs. The Nurse Practitioner also supports prevention services, health education, outreach initiatives, and collaborative care with healthcare and community partners.

Registered Nurse (RN)

Provides specialized nursing care through client assessments, testing, treatment support, education, follow-up, and health promotion. The Registered Nurse works collaboratively with the interdisciplinary team to support prevention, client engagement, outreach activities, and coordinated care while helping individuals navigate specialized HIV, HCV, and STBBI services.

Registered Practical Nurse (RPN)

Supports the delivery of specialized clinical services through client assessments, testing, treatment support, vaccinations (where applicable), education, and follow-up care. The RPN works closely with the interdisciplinary team to promote client engagement, outreach, and continuity of care.

Linkage to Care Coordinator

Supports clients in navigating healthcare and community services by coordinating referrals, reducing barriers to care, and connecting individuals with appropriate health and social supports. The Linkage to Care Coordinator plays a key role in client engagement, outreach, partnership development, and ensuring clients remain connected to the care they need.

Social Worker

Provides counselling, psychosocial support, crisis intervention, and system navigation to help clients address challenges that may impact their health and well-being. The Social Worker collaborates with the clinic team and community partners to strengthen access to resources, promote client engagement, and support overall wellness.

HIV/HCV Peer Support Worker

Draws on lived and living experience to provide compassionate peer support, encouragement, education, advocacy, and practical assistance. The HIV/HCV Peer Support Worker helps reduce stigma, build trust, and empower clients while supporting outreach, community engagement, and connection to healthcare and community resources.

Clinic Support Worker

Helps create a welcoming, inclusive, and supportive experience for everyone accessing the clinic by assisting with clinic operations, appointments, client engagement, and administrative processes. The Clinic Support Worker also contributes to outreach initiatives, community engagement, and ensuring clients feel supported throughout their visit.

Every member of the Gilbert Centre Clinic contributes to creating a welcoming environment where people feel respected, supported, and empowered. Whether providing clinical care, education, outreach, system navigation, or building community partnerships, our team works together to improve access to specialized HIV, HCV, and STBBI services across Simcoe Muskoka.
